Your first visit to our chiropractors
When you enter our chiropractic clinic waiting room, you'll notice that our entire office is designed to make you feel completely comfortable. Below are the intake forms that you'll need for your first visit at Unlimited Health. We appreciate it greatly when patients print the forms from the website and bring them along, as it decreases the amount of time you'll have to wait. Though if you do choose to fill out the forms at our office, it gives you more time to enjoy the fireplace and relaxing atmosphere!
During your first visit, we review your health history, go over your specific complaints and then perform a comprehensive exam. We discuss with you what we have found, your treatment plan and costs of treatment. Then you'll receive gentle adjustments to your spine and extremities to get your joints moving and aligned properly. You'll then schedule your next visit within two to three days to ensure that your body is responding well to the care. Your first visit should take between 15 and 25 minutes.
